1) Take the bumper off
2) Take the step plate off the bottom of the bumper
3) Loosen the brackets that are on the bumper and move them inward
so they will be on the inside of the frame rail not outside. (The
brackets are slotted. They should move easily like mine did.)
4) Hold the bumper to truck at desired height and mark with pen
where the top of the frame rail touches the brackets
5) Cut a 1/4" slice about an inch long out of the bracket
(this slot will allow the top bend of the frame to slide into the
brackets to get it close to the truck.
6) There will be a tab of metal on the frame that lines up with
the bottom hole of the bracket. Drill this out. (Forgot drill bit
size, but it's big enough to reuse the bolts that you first took out.)
7) Attach bumper with stock bolt/assembly through both holes and
washers between the supports to the side.
If you want to (I didn't), you can put the step plate back on too.
I actually mounted mine up even a little higher. There is about a
1/4" gap for the rear gate. I think it looks better that way.

And now for the pics (sorry didn't have time to take it off again for more
detailed pics.):
This shows the new hole drilled in the frame and the area I
slotted for a real sturdy bumper. Even with one bolt in each side
you can still use a high lift jack under it!

This shows the small gap left behind where the upper and lower
skids come together. It's nothing compared to the other gap that was
above the bumper.
